Food Handlers

Please complete and submit this online form to apply for a Food Handler Establishment license.

Mail or hand deliver a check for the appropriate fee to Wantage Board of Health, Wantage Municipal Building, 888 State Highway 23, Wantage, NJ 07461.  Wantage does not accept online payments or credit cards.

Food handler licenses are issued for one year, six month, or for one day.
The license fees for 2025 are $115 for an annual or seasonal license; $5 per day for a daily license.
